Before End Users can login, they must have a record in SAM.
End User Passwords
Passwords can be set on Public Forms to make an exact connection to a record in SAM. Previously entered data will be pre-filled.
There is one password per Family/Foster Family record. When there are multiple parents/providers, these end users will share a password to enter their record.

Public forms and Checklists set as view will always require that clients login. This is what end users see when they access a SAM Public Form to login.

How End Users Retrieve a Forgotten Password
1. The end users should open the public form
2. Click "Need of forgot your password?"
3. SAM will direct the user to a form to reset their password.
4. End users should enter an email address on record and the Name requested, then click Submit.

5. An email will be sent to the email address from your agency's Default Email Sending email account.
6. End users should follow the email to reset their password.
7. SAM directs end users to a form to input a new password and confirm the password.
8. Click 'Reset Password' when complete.
9. The end user will be directed to the original form to login with new credentials.
Troubleshooting Password Reset Issues
- Did the Password Reset email get sent?
- Is there a Sending Error? - verify your Default Email Account is authenticated.
- Is the message in the end user's spam folder?
- Verify the end user's name and email are on their client record in SAM.
- These record types share a password. Ensure end users accessing a Family/Foster Family record are sharing their password when they reset passwords.
Public Form Password Reset Messaging
When a password reset is requested from a Public Form, SAM will email the user instructions. This messaging can be modified from the Public Forms Links screen.

In SAM, click Settings (gear icon) > Public Forms Links > click Advanced Options (sidebar) > an option below to change the text the Public Form user will receive

The messaging shown to a form user at the Login screen can be modified.
- Open Form Customizer > Advanced Options > Advanced Form Properties
- Find the Login - Public Forms section
- Enter the "Login/Prefill Option Section Text/Question" with a new message > Click Save

Public Forms | Prompt User to Login
When Public Forms | Prompt user to login when duplicate email is enabled, SAM will prompt end user to login through a popup if a duplicate email address is entered into a public form. By default, this setting is enabled on all Public Forms.
End users can:
- Login
- Set/reset their password after clicking Forgot or Don't have Password
- Ignore the login prompt and add a new, potentially duplicate, client record
Disable Prompt End User to Login
For each form, SAM Leads can disable the login prompt.
