The Family, Child and Birth Parent records all have a Case which tracks their interaction with your program from Inquiry/Referral to case closure. Each case consists of several stages, some of which are processes within themselves, like the home study and adoption.
Adoption records connect the triad.
Family, Birth Parent and Child fields on the adoption table are interconnected, so an entry into one record will be reflected on the linked records.
Adoption records follow the adoption journey.
A new adoption record in SAM should be added when a Family matches with a Birth Parent or commits to a Child. An adoption record is required for each Child, so each sibling will have a unique adoption record.
Family, Child and Birth Parents can each have multiple adoption records to account for changes once a commitment or match to pursue adoption is made.
On an adoption record, Adoption Status tracks commitments, placements, commitment cancellations, dissolutions or disruptions.
Example Scenarios
Example: A Birth Parent decides to parent their child after matching with a Family, set the adoption status to commitment canceled or disruption. Create a new adoption record if the Family matches with another birth parent or child.
Example: A birth mother places multiple children for adoption. Each child will have an adoption record when matched with a Family.
Example: If a disruption or dissolution occurs for an adoption, add a new adoption record when another Family commits to adopting the Child.
Add an Adoption Record
Adoption records can be added from the Family, Birth Mother or Child cover page.

In SAM, open Adoptive Family record > click Adoptions (sidebar) > click Add Adoption (sidebar) > enter Birth Mother and/or Child and Commitment/Match Date > Save In SAM, open Child record > click Adoptions (sidebar) > click Add Adoption (sidebar) > enter Family and/or Birth Mother and Commitment/Match Date > Save In SAM, open Birth Parent record > click Adoptions (sidebar) > click Add Adoption (sidebar) > enter Birth Mother and/or Child and Commitment/Match Date > Save |
Adoption records entered erroneously can be deleted.
Move Adoption Record to New Case
For various reasons, an Adoption record may be under the incorrect case. Users can change the case linked.

Maintain records of potential matches and referrals of children to support matching a child and prospective adoptive family.
