Tables and Fields are fundamental to creating Forms, Reports, Auto Actions, and Templates.
Forms are used to view and enter data into SAM. Form Customizer is the tool that allows SAM Leads to edit and build forms. There are some concepts and terms to be aware of to help you navigate this tool.
Form Types
Categories to describe forms that you'll need to understand when editing or building your forms.
View Forms
- A sidebar is only available on view forms.
- Fields on view forms can be edited one at a time.
- Some fields combine data for better display, such as 'All Phone Numbers' or 'Full Address.'
Edit Forms
- Fields are not combined: Street Address, Street Address Line 2, City, State, Zip.
- SAM cannot display 'view' fields on edit forms, such as Timeline, Calculated Age, Length of Stay, Full Address.
Cover Page Forms
- It's possible to configure different Cover pages for main entities through Cover Page Redirect settings, should you have multiple requirements per program.
Public Forms
Form Layouts
- Vertical - Fields can be grouped in sections. Sections can be setup in a one or two column display.
- Horizontal - Multiple fields per row with multiple rows.
Main / Container Forms
The majority of forms in SAM are a Main/Container form. Main forms are based off Main entity or Case tables and can contain Sub-Forms. Container forms are generally created when building a Multi-record Sub-Form to hold the form.
Sub-Forms
Sub-Forms display data for Multi-record tables or for formatting information. Sub-Forms are defined with a dashed line encompassing the fields that make up the Sub-Form. Sub-Forms may be the only thing on a Main form.
Rules for using Sub-Forms:
- Sub-Forms are added to Main/Container forms.
- Sub-Forms display beneath fields on the Main/Container form.
Formatting Sub-Forms
To edit a Sub-Form, click 'Edit this Sub-Form' (bottom left of the form's display).
Add a Sub-Form
In SAM, open Form Customizer for the Main/Container form > click Advanced Options (sidebar) > Add Sub-Form > select desired Sub-Form > Save |
Available Sub-Forms relate to the main entity a form is based on. If a suitable Sub-Form does not already exist, you can create a new one.
Create New Sub-Form
In SAM, open Form Customizer for the Main/Container form > click Advanced Options (sidebar) > Add Sub-Form > click Create New Form (under Save) > select the table the new Sub-Form will be based off > set Form Properties as requested > Save |
Reorder Sub-Forms
In SAM, open Form Customizer for the Main/Container form > click Advanced Options (sidebar) > click Edit Sub-Forms Order > set order as needed |
Delete / Remove Sub-Forms
In SAM, open Form Customizer for the Main/Container form > click Advanced Options (sidebar) > click Remove Sub-Forms > click Remove (top right of Sub-Form) This function removes the Sub-Form from the Main/Container form, but does not delete the form in SAM. |
