Public Forms allow you to share SAM forms to users without access to your system. This includes clients, constituents and people referred to your organization. Forms are completed online and responses go directly into your SAM site in real-time.
Public Forms are commonly used for Inquiries, Family Applications, Employee Applications and Questionnaires.
These forms can be added to your website or shared via email with the form's URL.
Access Public Forms in SAM
Enabled Public Forms can be found at Settings > Public Forms Links.

Downloadable Family Portal Home Images
Downloadable messaging images for your family portal public form for you to use in your family portal header. Simply right click on the image and click "Save Image As" then save to your computer.
If you're creating these images in Canva or another design program, make sure to create images that are sized 750x350 pixels.
