Knowing your client's experience with Public Forms is important so your team can support end users with navigating and completing forms you share with them.
End User Passwords
Passwords can be set on Public Forms to make an exact connection to a record in SAM. Previously entered data will be pre-filled. Public forms and Checklists set as view will always require that clients login.
Password Login
This is a view that end users see when they access a SAM Public Form to login.
How End Users Retrieve a Forgotten Password
1. The end users should access the PROPER public form login screen.
2. Click 'Forgot or Don't Have a Password' under Password.
3. SAM will direct the user to a form to reset their password.
4. End users should enter an email address on record and the Name requested, then click Submit.
Email Address/Name Combination
5. An email will be sent to the email address from your agency's Default Email Sending email account.
6. End users should follow the email to reset their password.
7. SAM directs end users to a form to input a new password and confirm the password.
8. Click save when complete.
9. The end user will be directed to the original form to login with new credentials.
Troubleshooting Password Reset Issues
- Did the Password Reset email get sent?
- Is there a Sending Error? - verify your Default Email Account is authenticated.
- Is the message in the end user's spam folder?
- Verify the end user's name and email are on their client record in SAM.

The messaging shown to a form user at the Login screen can be modified.
1. Open Form Customizer > Advanced Options > Really Advanced Options > Advanced Form Properties
2. Find the Login - Public Forms section
3. Enter the "Login/Prefill Option Section Text/Question" with a new message. Click Save.
Public Forms & Auto-Matching
Public Form | Auto-Match
When Public Form | Auto-Match is enabled, records entered through the public forms are automatically matched to existing records by:
- Last Name + Email Address
- Last Name + Phone Number
- Email Address + Phone Number
When a new Public Form is submitted and SAM matches at least two of the criteria, the information captured on the public form will be entered for the matched record. When these criteria are not matched, a new record will be created.
When Auto-Match is enabled, SAM will prompt end user to login through a popup if a duplicate email address is entered into a public form.
End users can:
- Login
- Set/reset their password after clicking Forgot or Don't have Password
- Ignore the login prompt and a new, potentially duplicate, client record
By default, Public Forms have Auto-Matching disabled because it makes Public Forms less secure.
Disable Auto-Match
For each form, SAM Leads can disable the login prompt.
In SAM, click Settings > Public Forms > select desired form > click Advanced Options (sidebar) > Advanced Form Properties > under Public Form Settings, Set Public Forms | Prompt user to login when duplicate email = No This will create duplicate records if Public Form | Auto-Match is also set to No (default setting). We don't recommend changing this because security is more important than convenience. |
Public-Form Auto-Match Record Criteria
When Public Form | Auto-Match is enabled, SAM will use Public-Form Auto-Match Record Criteria to set when an auto-match should occur. By default, the Criteria is "Login Password" is Empty. This setting is done on each Main Entity Table.
To set the Public-Form Auto-Match Record Criteria
In SAM, click Settings > Tables and Fields > select desired table > click Advanced Options (sidebar) > Really Advanced Options > Public-Form Auto-Match Record Criteria > update criteria as desired > Save |
If you want all records to Auto-Match regardless of any criteria, delete all the filters.
If there are Duplicate Records, SAM will login (or reset the password) to the latest one.
